What We're Looking For:
- Valid California LVN or RN license
- Proven experience as an MDS Coordinator in a skilled nursing facility
- Deep understanding of PDPM, care planning, and state/federal regulations
- Ability to collaborate across interdisciplinary teams with clarity and confidence
- Strong organizational and documentation skills
