Case Manager (Maternity) (Healthcare)
Performs a variety of services related to the case management of patients.
Identifies needs of patients and draws from a comprehensive bank of community resources.
Performs the CPSP initial assessments, gathers baseline data, and asks questions designed to identify issues affecting the patient's health and the pregnancy outcome, their readiness to act, and resources needed to address the issues.
Develops an individualized care plan (ICP) and identifies interventions to meet the patient's unique needs.
Performs the CPSP second and third trimester reassessments to check the patient's progress on the issues they want to change and identify if new issues have arisen for the patient and their family.
Completes the CPSP Postpartum assessment and care plan to identify various issues that may arise, including difficulties breastfeeding, postpartum depression, and making arrangements for management of ongoing chronic conditions.
Respects and values the patients in showing sensitivity to issues of diversity, ethnicity, gender, sexual orientation, age, and disability.
Engages the patient in the development of a service plan tailored to meet the patient's unique needs, circumstances, and preferences as determined through assessment.
Arranges, coordinates, and monitors services from qualified providers that are prompt, adequate, and appropriate.
Educates the patient to make informed choices; maintains client records that include assessment plans, and case notes that are comprehensive and up-to-date.
Organizes the case management systems including intake and patient record management develops educational programs based on identified needs.
Participates in interdisciplinary treatment team and supports patient's ability to communicate his/her needs effectively.

KNOWLEDGE, SKILLS, AND ABILITIES
Knowledge of mental illness issues and sensitivity towards issues of diversity.
Ability to work autonomously and seek out consultation from peers and others when needed.
Participates in planning and evaluation of case management program.
Can express the mission of SCHC to other staff, patients, and to the broader community.
Knowledge of adult learning theories and group dynamics.
Excellent verbal and written communication skills.
Computer literacy.
EDUCATION & EXPERIENCE
High school diploma or equivalent.
One year full-time paid practical experience in providing perinatal care.
Current CPR certification required.
PREFERRED QUALIFICATIONS
Two-Year Associate Degree or equivalent experience in Case Management.
Previous experience working in an outpatient clinic or doctor's office.
